Abstract
Mental healthcare faces a dual crisis: a global treatment gap leaving most people without support, and an overly biomedical orientation that proves inadequate for communities where spirituality is central to wellbeing. Meanwhile, generative AI has become part of routine clinical practice, with both patients and practitioners using these tools to make sense of diagnoses and evaluate treatment options. This paper argues that governing AI in mental health is not a technical problem awaiting definitive solution but a divergent problem requiring ongoing wisdom to navigate tensions between access and quality, efficiency and relationship, technological capability and human presence. We propose multiplex philosophy as a framework for this navigation, drawing on Islamic conceptions of dynamic balance and employing an objectives-based (maqāṣid) methodology to establish five anchor principles for responsible AI integration: preservation of human dignity, protection of intellect, promotion of benefit and prevention of harm, maintenance of human accountability, and cultivation of balance. We identify a fundamental problem with current AI approaches: the ontological flattening loop that reduces humans to mechanism while inflating machines to agency. Applying this framework to Traditional Islamically Integrated Psychotherapy (TIIP), we demonstrate how AI can empower practitioners and patients while preserving irreducibly human functions of spiritual guidance, crisis intervention, and the therapeutic relationship. We argue that spiritually grounded frameworks like TIIP can strengthen responsible AI integration, offering resources urgently needed as technology reshapes mental healthcare.
